Effective July 17, 2012, the business you knew as Crucial Technology became Micron Consumer Products Group, a Division of Micron Semiconductor Products, Inc. Crucial.com is a brand of Micron Consumer Products Group, and as such, all resale certificates submitted for review need to reflect Micron Consumer Products Group, a division of Micron Semiconductor Products, Inc. as the name of the seller. We kindly ask you to update all of your records accordingly and to address all future business correspondence to our new name. If you are limited to a certain number of characters on your form, please use Micron Consumer Products Group, a Division of MSP.
In order to comply with state sales and use tax laws, we are required to have proof of exemption on file. This documentation must be submitted, reviewed and entered prior to your order being shipped.
For your convenience, we have provided acceptable forms or links to acceptable forms for each state that provides on-line forms. Please select the correct certificate for your ship-to state and exempt reason. If the form that you need is not included on this page, you should consult your tax authorities to obtain the proper documentation. The form should be printed, signed, and faxed back to Micron Consumer Products Group at 1-208-363-5633.
Important: Please write your web order request number on the cover sheet of your fax so we can process your request in a timely manner.
Be sure to fill out the form entirely, or the shipping of your order will be delayed until we receive a complete form.
Orders indicated as tax exempt, not supported by the required documentation will experience delays in processing. In order to expedite your order, please provide the exemption information as soon as possible.
